Medicaid 1115 Waiver
Under this waiver, states partner with established Social Care Networks (SCNs) to connect members with the services they need most.
One of these networks is the WNY Integrated Care Collaborative (WNYICC) — a hub that brings together community‑based organizations and government agencies. WNYICC provides care management and helps guide Medicaid members to the right supports at the right time.
The target population includes Medicaid members who are:
- Experiencing health‑related social needs
- High‑risk or high‑need
- Living with disabilities
- Managing serious health conditions
- Facing significant social or environmental challenges
The Goal
To improve the overall well‑being of Medicaid recipients by integrating social supports with health services. This includes expanding eligibility, addressing social determinants of health, promoting health equity, and reducing disparities across our region.
Buffalo Go Green’s Role: Nutritional Supports
Buffalo Go Green, Inc. is proud to serve as a provider of nutritional support services within the waiver that is helping increase food access and nutrition education in Buffalo. Our offerings include:
- Produce Prescriptions
- Pantry Stocking for Vulnerable Populations
- Nutritional Counseling
- Medically Tailored Meals (coming soon!)
Each week we package and send out up to 150 clinically appropriate food boxes, utilizing the Medicaid 1115 waiver benefits. Our team assembles food boxes for eligible members of our community. Each box contains enough fresh produce and pantry staples to prepare 21 meals per week created by a registered dietitian from our team, and a partnered professional chef.
In partnership with Urban Fruits and Veggies, we grow fresh, sustainable produce right here in Buffalo. We’re dedicated to bringing our community sustainable, nutrient-rich produce while protecting our environment.
Our Produce Prescription program is Buffalo Go Green’s largest and longest‑running initiative, growing steadily since 2017.
